What breaks, and when
The agency management system is the business. When it is slow, every quote is slow; when it is down, nobody can bind. Most agencies run it on a server that was sized for half the current staff, or on a hosted version nobody has tested restoring.
Carrier portals each want their own login, their own browser quirks and their own multi-factor method. Staff end up with passwords on sticky notes, which is exactly what your E&O carrier's questionnaire asks about.
Acquisitions. Independent agencies grow by buying other agencies, and every one comes with its own systems, its own email domain and years of policy history that an underwriter may one day need.
Remote producers. The agency has an office, but half the book is written from kitchens and cars. Home offices set up in a hurry are the weakest link in the agency's security.

We have merged agencies. Three businesses, three accounting systems and nineteen years of records into one, after three other IT companies could not produce a plan. We know which data has to migrate and which can be archived readable, and we know the calendar is set by renewals, not by IT.
We treat the E&O and cyber insurance questionnaire as a project rather than a form. Multi-factor everywhere, backups with tested restores, and documentation that lets you answer yes honestly.
And we set up home offices properly, on managed devices, so a producer working from home is as secure as one at a desk in the office.

Cyber and E&O carriers now ask specific questions: multi-factor on email and remote access, endpoint detection, tested backups, a written incident plan, security awareness training. Answering no can raise the premium or void a claim. We build each of those controls, keep the evidence, and fill in the questionnaire with you at renewal. For agencies handling health or financial data, we align the setup to the state privacy rules and the carrier contracts that require it.
